RESUMO

The authors have proposed and tested a new noninvasive technique of Doppler ultrasound study of impaired blood flow in the segmental arteries, which indirectly evaluates spinal cord hemodynamics in vertebral column diseases. Clinical trials of the technique proposed have provided evidence for the association of blood flow in the segmental arteries with the degree of vertebral column canal and kyphotic deformities. A combined impact of spinal deformity and impaired blood flow in the segmental arteries at the level of lesion on the severity of neurological disorders and on deterioration of life quality in patients with spinal inflammatory diseases has been found. A relation of impaired arterial blood flow of the lower extremity to circulatory changes in the segmental arteries has been established in spinal diseases.

RESUMO

The clinical and immunological features of tuberculous spondylitis are analyzed in adult patients. Based of the results of examination of 92 patients, the authors consider that all the patients may be arbitrarily divided into 2 groups according to the body's responsiveness and compensatory capacities. Group 1 patients have a 1-2 year history of tuberculosis, their T- and B-lymphocyte sensitization being less pronounced. These patients are compensatory, as evidenced by the high rate of formation of epidural abscess and by elevated central hemodynamic parameters. Group 2 patients are highly sensitized with tuberculous antigen in the T and B immunity systems. In these patients, the specific process is accompanied by a more pronounced spinal cord compression. At the same time, more steady-state central and peripheral hemodynamic parameters are indicative of the development of compensatory mechanisms just in these patients.

RESUMO

The paper deals with the study of central and peripheral hemodynamic features in patients with multiorgan tuberculosis. The study is based on the analysis of examination findings and treatment outcomes in 42 patients with concurrent tuberculosis of the lung and vertebral column. A control group consisted of 46 patents with spinal tuberculosis. The authors provide evidence that concurrent pulmonary and spinal tuberculosis substantially makes basic central and peripheral hemodynamic parameters worse in patients with tuberculosis spondylosis, aggravates their general condition, requires cardiotropic therapy on an individual basis, substantially prolongs preoperative preparation and postoperative rehabilitation.

RESUMO

The authors study features of development of spinal cord compression, neurological affections and quality of life in patients with polyorganic tuberculosis. As shown by examination and treatment of 72 patients having pulmonary and spinal tuberculosis vs 46 control patients with spinal tuberculosis only, patients with pulmonary and spinal tuberculosis demonstrate faster development of spinal cord compression, neurological disorders, more severe and faster loss in motor activity and quality of life than patients with isolated spinal lesion.

RESUMO

The paper considers whether a new impedance measurement technique can be used to estimate the activity of the process. Following 6 hours of intradermal tuberculin administration, the permeability of capillaries proved to be of the maximum informative value, which provides not only a judgement of the tuberculous origin of the disease, but evaluates the activity of the process, by allowing for the inclusion of this technique into a package of diagnostic measures in patients having the suspected origin of the disease.

RESUMO

A new noninvasive impedance method has been proposed to determine the permeability of skin capillaries, which involves measurement of the electric resistance of a skin area before and after exposure to negative pressure. There is a convergence of clinical data and simulation findings. The changes in the permeability coefficient were studied in different periods after recovery of adequate blood supply in the diseased limb. The investigation has allowed the developed permeability coefficient to be included into the algorithm of diagnosing and treating patients with arterial diseases.

RESUMO

The paper outlines a procedure for evaluating distal extremity microcirculation, which consists in exposing the examined skin area to negative pressure and in recording the electric resistance of the area in question prior to and after exposure, the permeability of skin capillaries is judged from the value differences. Its predictive value was assessed in patients with terminal lower extremity arterial ischemia.

RESUMO

Experience with treatment of 410 patients with obliterating diseases of lower extremities arteries with the terminal stage of ischemia and occlusions of the femoro-popliteal segment have shown that successful results of small amputations can be obtained but against the background of complex treatment based on a thorough and comprehensive examination of the patient, and aimed at the recovery of adequate blood supply of distal parts of the extremity by operations of direct and indirect revascularization, normalization of microcirculation and struggle against infection. It allows performing amputation distal to the knee joint in 79.5% of cases, and to reduce the amount of postoperative complications in 38.8% of cases.
